چکیده

MOTS CLÉS Cardiomyopathie induite par le stress ; A 73-year-old woman without previous medical history was admitted for suspected persistent thoracic anginal pain following a violent suffocating cough. On admission, results of physical examination and per-critical electrocardiogram were normal. Aortic dissection and pulmonary embolism were ruled out by computed tomography scan. The troponin peak reached 3.55 g/l at day one, while an electrocardiogram showed T-wave inversion in the anterior and lateral leads. A transthoracic echocardiogram showed akinesis of the midportion of the lateral, posterior, anteroseptal and anterior walls of the left ventricle while the apical and basal parts of these walls were normocontractile (Panel A) and left ventricular ejection fraction was at 40%. Coronary angiography was normal. Cardiovascular magnetic resonance (CMR) confirmed echocardiographic findings with wall motion abnormalities confined to the midportion (Panel B). High T2 signal was observed within the asynergic myocardium (Panel C) while no regional delayed gadolinium hyperenhancement

Evaluation of tissue doppler echocardiography and T2* magnetic resonance imaging in iron load of patients with thalassemia major

Background: Iron-mediated cardiomyopathy is the main complication of thalassemia major (TM) patients. Therefore, there is an important clinical need in the early diagnosis and risk stratification of patients. The aim of this study was to evaluate the efficacy of tissue doppler imaging (TDI) to study cardiac iron overload in patients with TM using T2* magnetic resonance (MR) as the gold-standard...

Hypertrophic cardiomyopathy with midventricular obstruction and apical aneurysm formation in a single family: case report

BACKGROUND Hypertrophic cardiomyopathy (HCM) is an extremely heterogeneous disease. An under recognized and very often missed subgroup within this broad spectrum concerns patients with left ventricular (LV) apical aneurysms in the absence of coronary artery disease. CASE PRESENTATION We describe a case of HCM with midventricular obstruction and apical aneurysm formation in 3 patients coming f...

Predictors of Outcome After Alcohol Septal Ablation for Hypertrophic Obstructive Cardiomyopathy: An Echocardiography and Cardiovascular Magnetic Resonance Imaging Study.

BACKGROUND Alcohol septal ablation (ASA) is used for treatment of medically refractory hypertrophic obstructive cardiomyopathy patients with severe left ventricular outflow tract (LVOT) obstruction. The current recommendations restrict the procedure to adults with favorable anatomy and no other coexisting surgically amenable disease. Some patients remain symptomatic with residual LVOT obstructi...

Myocardial fibrosis in desmin-related hypertrophic cardiomyopathy

Desmin-related myopathy (DRM) is known to cause different types of cardiomyopathy. Late gadolinium enhancement cardiovascular magnetic resonance (CMR) has been shown to identify fibrosis in ischemic and non-ischemic cardiomyopathies. We present a rare case of desmin-related hypertrophic cardiomyopathy, CMR revealed fibrosis in the lateral wall of the left ventricle. CMR is superior to conventio...

Delayed hyperenhancement in a case of Takotsubo cardiomyopathy.

Takotsubo cardiomyopathy (TTC) consists of an acute onset of transient akinesia of various parts of the left ventricle (apex and mid in classical TTC, mid and base in the variant form), without significant coronary artery stenosis, often accompanied by chest pain, dynamic reversible ST-T segment abnormalities and elevation of cardiac enzymes disproportionate to the extent of akinesia. Contrast-...
